General Description

Mitragynine is an indole-based alkaloid, abundantly present in the Southeast Asian plant Mitragyna speciosa, commonly known as kratom. With a concentration ranging from 0.5 to 1.5% in dried leaves, mitragynine has been identified as a partial opioid agonist, acting on µ (MOR), δ (DOR), and κ (KOR) receptors. It produces stimulant effects in low doses and sedative effects in high doses. First isolated in 1921, its chemical structure was fully elucidated in 1964.

Uses Known for its analgesic properties, mitragynine's action on opioid receptors makes it comparable to morphine. It has garnered interest for potential medicinal uses, including treating opioid withdrawal and chronic pain. We describe a death attributed to mixed drug toxicity—primarily mitragynine. The current report revealed the detection of mitragynine by routine toxicology screening, describes a specific gas chromatography–mass spectrometry selective ion mode (GC–MS SIM) analysis and presents the postmortem concentrations in peripheral blood, central blood, liver, vitreous, urine and gastric contents.
